Program Name: CHESS Health SMS Messaging Program

Program Description
If your healthcare provider, treatment program, or health plan, uses CHESS Health’s eRecovery Solution, you may receive SMS (text) messages from CHESS Health, sent on behalf of your provider. These messages are related to your care, treatment, or services and may include reminders, check‑ins, updates, and general supportive or informational messages. Messages are not used for marketing purposes.

Consent
By providing your mobile phone number and opting in, you consent to receive SMS text messages from CHESS Health on behalf of your provider. Messages may be sent using automated or prerecorded technology. Consent is not a condition of receiving care or services.

Opt‑Out Instructions
You can cancel the SMS service at any time by texting STOP to any message. After you send STOP, you will receive a confirmation message indicating you have been unsubscribed. You will no longer receive SMS messages unless you opt in again.

Your privacy matters to us. Information collected through this SMS messaging program is used only to support your provider’s services and is handled in accordance with our SMS MEssaging Privacy Policy. CHESS Health does not sell or share SMS consent with third parties for their own marketing purposes.

Eligibility
You must be at least 18 years old, or have permission from a parent or legal guardian, to participate in this SMS messaging program.

We may update these SMS Messaging Terms & Conditions from time to time. Any changes will be effective when posted.
