Closed Loop Referral

Closed loop referrals send, track, and manage referrals to community providers and agencies for substance use, mental health, and health-related social needs (HRSN), such as housing, food security, and transportation. A closed-loop referral system confirms that a person actually reached the service they were referred to, not just that a referral was made. Traditional referrals end at the handoff.
