Adult Alcohol Use Self-Assessment
SBIRT (Screening, Brief Intervention, and Referral to Treatment) is a comprehensive, evidence-based approach designed to identify, reduce, and prevent problematic substance use. Most commonly, SBIRT is done, often poorly, in person during primary care office visits.
This demo is an example of a short self-assessment on alcohol use, which personalizes feedback and interventions based on the user’s risk level. Low-risk individuals receive positive reinforcement and encouragement to drink less; medium-risk individuals get stronger motivation and tools to reduce drinking; high-risk individuals are strongly encouraged and supported to seek treatment and recovery. This approach enhances screening, delivers more effective interventions, and generates additional data.

Youth Alcohol and SUD Quiz
In some cases, asking individuals to self-assess isn’t the best prevention strategy. For example, to educate adolescents, a better approach can be through an interactive quiz that delivers personalized feedback and content.
In this example, we developed an engaging, easy quiz for youth about alcohol and substance use that sparks curiosity and promotes critical thinking. It encourages honest self-assessment, empowering young people to make informed decisions and reduce the risk of substance misuse. By using “smart interventions,” the quiz tailors responses based on individual answers, offering personalized educational resources, videos, and links to community support tools.

Mental Health Assessment
Quizzes and assessments can be designed to capture data and provide recommendations on a range of topics beyond substance use disorder.
In this sample, the assessment focuses on mental health and aims to connect individuals with mental health services and suicide prevention resources. Individuals experiencing mental health challenges are invited to complete a brief questionnaire derived from the PHQ-2 and PHQ-9 scales, which includes a question about thoughts of self-harm. Based on their responses, participants receive personalized recommendations for local and national services and resources.

Fact or Myth: Nicotine Quiz
Who doesn’t enjoy playing Fact or Myth? In this example, the quiz is designed to educate youth about various forms of nicotine use, including vaping.
The quiz features questions that assess the impacts of nicotine on health and well-being. Participants receive immediate feedback on their answers, helping to confirm or correct their understanding. They also gain access to additional resources for further exploration. This interactive approach empowers young individuals with knowledge about nicotine, encouraging informed decision-making regarding its use.
